Decide what change you want to see
Specify the starting condition, the people or institutions to be reached, the intended improvement and when it will be assessed. Keep an output, such as installing equipment, separate from an outcome, such as regular access to a working service.
Collect proportionate evidence
Use the agreed indicators, field reports, receiving acknowledgments and follow-up checks. Protect personal information and obtain publication consent before sharing photographs or beneficiary stories.
Plan continued use
Identify who owns the asset, who operates it, who funds running costs and how problems will be addressed. At closure, document what was achieved, what remains uncertain and what needs follow-up. Published success stories should reflect supported facts, not promotional assumptions.
