What the Collector and company should notice
The score is 77.50%, yet 12 children in this fictional measured cohort still meet the adopted SAM definition. Their clinical follow-up remains a priority regardless of the composite. Adult screening/referral also has 200 incomplete service units.
01 · UNDERSTAND THE GOAL
What belongs in this SDG page?
Child immunisation, maternal care, institutional delivery, adult prevention/referral pathways, dependable village outreach and linked child nutrition follow-up.
Full official goal scope · what is not measured here
The six local factors illustrate selected service pathways. The topics below are broader: a link to a topic does not mean its official target or outcome has been measured. A 100% scenario closes these six local targets only.
Coverage boundary This is a local service diagnostic, not the WHO UHC index or the full SDG 3 assessment. Mortality, communicable diseases, mental health, financial protection and other outcomes require additional data. Nutrition is primarily linked to SDG 2.2 and is included here as a cross-sector health concern.
Equity check Ask for safe aggregate coverage by sex, age, disability and remote habitation. Maternal and child records remain in departmental systems. A good composite never removes the duty to follow up an individual high-risk case.
02 · SHOW THE PERCENTAGE AND ITS BASIS
Six measurements. One transparent score.
The current score is calculated from measured service rates and adopted weights. Every figure is traceable to a defined numerator, denominator, date, source and reviewer. The sample uses equal-size comparable-period cohorts to make the arithmetic easy; real returns must reconcile actual cohort changes.
03 · WHAT EXISTS, WHAT IS REQUIRED, WHAT IS STILL OPEN
Gap analysis a company can act on.
Service achievement, confirmed planned work and open needs are separate. Planned units are not included in the current achievement percentage. Gap units use each indicator’s own cohort and must not be added as unique beneficiaries.
Illustrative, not district statistics. Percentages and counts describe different units; see each definition.
Resident children completing the age-appropriate schedule due by the reference date, under the department-approved definition.
Required universe
All 100 resident children due that same schedule, including missed and migrating children after reconciliation.
How to measure
Use the due-list and vaccination register; reconcile residence, age, duplicate records and actual doses received. Planned camps are not completed immunisation.
When to report
Monthly due-list review, quarterly validation and end-of-cycle follow-up.
Responsible office
PHC Medical Officer, ANM and the block health reviewer.
Required evidence
Signed aggregate due-versus-completed return and register reconciliation; no names or child health cards in the public view.
Women completing the ANC contacts due in the period
+
What is counted
Resident women with all antenatal contacts due in the period completed under the approved schedule, including required documented referral follow-up.
Required universe
All 80 resident women due those contacts in the same reference period.
How to measure
Approve the schedule and due-cohort rules; reconcile the maternal register, completed contacts and referral closure. This is a reporting example, not a prescribed clinical schedule.
When to report
Monthly tracking; quarterly signed aggregate validation.
Responsible office
Medical Officer, ANM and the maternal-health programme reviewer.
Required evidence
Aggregate due-contact completion and referral follow-up statement, independently checked by the authorised officer.
Institutional deliveries in the resident delivery cohort
+
What is counted
Resident deliveries occurring in an appropriate authorised institution during the reference period.
Required universe
All 60 resident deliveries in the same period, including those outside the local facility catchment.
How to measure
Reconcile resident pregnancy/delivery records and institution confirmations. A facility total may include non-residents and cannot substitute for a village denominator.
When to report
Monthly reconciliation and quarterly review.
Responsible office
PHC Medical Officer and the maternal/newborn programme team.
Required evidence
Aggregate resident delivery reconciliation and validated institutional-service confirmation.
Due adult prevention and referral pathways completed
+
What is counted
Eligible resident adults completing the adopted screening pathway, including follow-up for referrals due by the reference date.
Required universe
All 500 resident adults due that defined pathway in the period, not only camp attendees.
How to measure
The health office adopts the eligibility, tests and referral-closure definition. De-duplicate people and reconcile due referrals; counting tests or repeat visits is insufficient.
When to report
Monthly due-list updates, quarterly validation and six-month pathway review.
Responsible office
PHC/Health and Wellness Centre team and the district NCD reviewer.
Required evidence
Signed aggregate eligible-screened-completed-referral reconciliation. Patient results remain in the clinical system.
Resident children classified with severe acute malnutrition under the documented, approved clinical measurement protocol.
Required universe
All 200 children in the defined eligible measured cohort with valid comparable measurements; show measurement coverage separately.
How to measure
Qualified health staff apply the approved protocol and validate measurement completeness. The illustrative 20% worst anchor is an administrative scoring choice, not a clinical threshold. Never infer recovery from supplies distributed.
When to report
According to the clinical follow-up protocol; aggregate monthly review and comparable remeasurement.
Responsible office
ICDS/CDPO and authorised PHC/NRC clinical teams.
Required evidence
Validated aggregate measurement coverage, SAM classification counts and clinician-confirmed follow-up results. Individual case records remain confidential.
Each example work record links to one measured factor, a before/after count, source evidence and an independent review. The arithmetic explains the score change; it does not prove that a single project caused the entire change. Do not credit two projects for the same service unit.
EXAMPLE WORK RECORD+3.00 pp
Missed-child mobilisation and service follow-up
70baseline count→85current count
The comparable coverage count moved from 70 to 85; the department checks actual schedule completion.
Evidence and accountability
Required evidence: Signed aggregate due-versus-completed return and register reconciliation; no names or child health cards in the public view.
Responsible office: PHC Medical Officer, ANM and the block health reviewer.
The institutional-delivery count increased from 48 to 54 across comparable periods. This is a service-access indicator, not a maternal-mortality estimate.
Evidence and accountability
Required evidence: Aggregate resident delivery reconciliation and validated institutional-service confirmation.
Responsible office: PHC Medical Officer and the maternal/newborn programme team.
The illustrative rate falls from 10% (20/200) to 6% (12/200). The local normalized score rises from 50 to 70. Twelve current cases still need clinical follow-up.
Evidence and accountability
Required evidence: Validated aggregate measurement coverage, SAM classification counts and clinician-confirmed follow-up results. Individual case records remain confidential.
Responsible office: ICDS/CDPO and authorised PHC/NRC clinical teams.
The six non-overlapping indicator contributions reconcile to this score change. They are not six separate counts of unique beneficiaries.
Financial accountability stays separate
Illustrative earlier/current work ledger. Pending commitments do not count as delivered services, and expenditure does not automatically increase SDG achievement. The new open-need estimates below are separate from this ledger and need overlap checks.

OPEN SERVICE GAP 10 children
Support outreach for the remaining due children
Department-approved mobilisation and logistics around authorised clinical services. Health professionals retain all clinical decisions and delivery.
Unreserved indicative estimate₹1,00,000
Maximum modeled score gain+2.00 pp
When
90 days, linked to the official due schedule.
Required before starting
Signed aggregate due-versus-completed return and register reconciliation; no names or child health cards in the public view.
Risk to check
Cohort changes or counts of doses instead of fully covered children can overstate achievement.
After handover
PHC keeps the due-list and routine outreach responsibility.
Estimate scales proportionately for explanation; actual fixed and recurring costs need a separate approved estimate.
OPEN SERVICE GAP 12 women
Strengthen access and referral follow-up
Approved appointment coordination, access logistics and referral tracking. No clinical case files are shared with the company.
Unreserved indicative estimate₹1,80,000
Maximum modeled score gain+3.00 pp
When
Three months with monthly officer review.
Required before starting
Aggregate due-contact completion and referral follow-up statement, independently checked by the authorised officer.
Risk to check
Counting registrations or one visit as a completed due schedule would inflate performance.
After handover
The health department maintains the maternal-service pathway.
Estimate scales proportionately for explanation; actual fixed and recurring costs need a separate approved estimate.
OPEN SERVICE GAP 4 delivery pathways
Improve remaining delivery-access arrangements
Department-approved access/referral arrangements, emergency readiness and documented handover. Clinical care stays with authorised providers.
Unreserved indicative estimate₹80,000
Maximum modeled score gain+1.00 pp
When
90 days to establish arrangements; review each reporting period.
Required before starting
Aggregate resident delivery reconciliation and validated institutional-service confirmation.
Risk to check
A higher institutional-delivery rate alone does not prove quality of care or reduced mortality.
After handover
District health team maintains referral readiness and service availability.
Estimate scales proportionately for explanation; actual fixed and recurring costs need a separate approved estimate.
OPEN SERVICE GAP 150 adults
Support the unreserved prevention-service cohort
Approved access logistics, equipment readiness and tracking support around clinician-led services; verify referral completion.
Unreserved indicative estimate₹3,00,000
Maximum modeled score gain+6.00 pp
When
Six months, with monthly aggregate progress checks.
Required before starting
Signed aggregate eligible-screened-completed-referral reconciliation. Patient results remain in the clinical system.
Risk to check
Service completion is not a guarantee of disease control or a reduction in mortality.
After handover
Health department owns follow-up and equipment maintenance responsibilities.
Estimate scales proportionately for explanation; actual fixed and recurring costs need a separate approved estimate.
OPEN SERVICE GAP 3 sessions
Make remaining scheduled sessions dependable
Approved venue readiness, power, logistics and equipment support. Staff and clinical-service availability must be confirmed by the department.
Unreserved indicative estimate₹1,50,000
Maximum modeled score gain+2.25 pp
When
90 days, matched to the approved session calendar.
Required before starting
Approved schedule, signed delivered-service checklist and aggregate referral summary.
Risk to check
Adding low-service events must not replace the due-session denominator or lower the service standard.
After handover
PHC owns the continuing calendar and required staff deployment.
Estimate scales proportionately for explanation; actual fixed and recurring costs need a separate approved estimate.
OPEN SERVICE GAP 8 child follow-up pathways
Support department-led follow-up and nutrition logistics
Only approved non-clinical logistics and programme support around clinician-led care. Actual improvement needs repeat clinical measurements; this plan does not predict recovery.
Unreserved indicative estimate₹2,00,000
Maximum modeled score gain+2.00 pp
When
As directed by the clinical team; six-month programme review.
Required before starting
Validated aggregate measurement coverage, SAM classification counts and clinician-confirmed follow-up results. Individual case records remain confidential.
Risk to check
Missing measurements or a changing cohort can make prevalence look better. A high overall score never overrides clinical needs.
After handover
ICDS and health teams continue the approved care and follow-up pathway.
Estimate scales proportionately for explanation; actual fixed and recurring costs need a separate approved estimate.
06 · WHAT, HOW, WHEN AND WHO
From a register entry to an accountable company proposal.
01
Define what will be measured
Collector Office + department
Adopt the indicator, denominator/cohort, direction, target, anchor, weight, mandatory factors, geography and dates before measurement. Version changes; do not change the test after seeing results.
02
Enter departmental facts
Assigned departmental maker
Return village/LGD codes, numerator, denominator, actual source date, period, register reference, aggregate evidence, N/A/unavailable reasons and confirmed plan units. Personal records stay in the source system.
03
Independently verify
Different authorised reviewer
Reconcile the complete eligible cohort, duplicate records, measurement coverage, date freshness, plan overlap and evidence. Return discrepancies; missing values never become zero.
04
Approve a safe report release
Collector / authorised district approval
A real corporate-facing page needs a separately approved safe aggregate release. Do not expose the private SDG workspace, NGO grants, evidence or small identifiable cohorts. Publish the as-of date, coverage and method with the percentage.
05
Prepare a verified unmet-need report
Department / permitted NGO + PMU
Specify the exact village, open service units, existing schemes/plans, proposed intervention, site readiness, technical estimate, permissions, maintenance owner and outcome test. Route through normal need verification.
06
Agree the company contribution
Company + department + district approval
Confirm permitted delivery arrangements, milestones, responsibility and reporting dates. Keep committed, delivered/disbursed and utilized/verified figures separate. No example-page button makes a financial commitment.
07
Monitor delivery and hand over
Responsible line office + independent field review
Check actual service delivery and functionality, technical acceptance, receipts and maintenance ownership. Photographs and expenditure are supporting evidence, not substitutes for the outcome measurement.
08
Remeasure, then update achievement
Department → verifier → Collector
Use the adopted method and a reconciled cohort at the next due observation. Record the new date, evidence and approval. Only then replace the current percentage and publish the measured contribution with its limits.
VILLAGE NEED REPORT · MINIMUM CONTENT
The company pitch should answer these questions.
Where? Verified village/LGD, habitation/site and responsible office.
What exists? Current service count, denominator, rate, source date and evidence status.
What is needed? Target count − current achievement; subtract confirmed planning units for the open requirement.
Why this intervention? Root cause, readiness, permissions and alternatives.
How much and when? Independent estimate, recurring costs, delivery milestones and due dates.
What result? Specific service units, measurement method, independent reviewer and follow-up date.
Who sustains it? Named operating/maintenance owner and handover acceptance.
Village → Gram Panchayat → taluka → district
Keep verified LGD mappings. For an equity view, give each qualified village one equal weight and show qualified/total villages. Do not average GP or taluka averages without preserving the village counts. Separately aggregate numerator/denominator service coverage.
Example: one village at 70% and one unassessed village means “70% among 1 of 2 qualified villages”, not 35% district achievement. Keep the unassessed village in the action queue. A GP-level official PAI score must not be relabelled as a village score.
